Library - Incident Report Form

This article references an incident report form that is used for informing library employees about issues that occur in the library.

The welcome desk staff at the library are trained in security and can assist with some emergencies or alarms that are triggered. This incident report form is used to create a record of the event and alert the proper authorities for course of action.


The library report form can be used for problem patrons, this form allows a record to be established regarding the problem patron.
